Please call to arrange a prenatal visit if you are a new parent-to-be. This allows time for
you to meet with one of the doctors and ask any questions you may have regarding the
practice and about the care of your newborn.

Your First Appointment...
Please bring any records from previous doctors to your first appointment and most
recent insurance card. To help speed up the proccess you can print and complete
the two Forms below (one for each patient) and bring them to your first appointment.
Leaving the Practice...
Patients leaving the practice and would like a copy of their records have 2 options.
OPTION 1: Summary of records that only includes your most recent physical, Immunization
Records and Front Sheets at NO COST.
OPTION 2: Copy of your Complete medical records for $15. You must print and complete a
Record Release form before we send you a copy of your records which can be downloaded
in the link below. We accept cash, credit or checks. (make checks payable to Lynnfield
